    ACCUSED JULIE MAR DANTE y MASAMLOC, through


    Counsel, unto the Honorable Court, most respectfully avers that:

    1. Accused JULIE MAR DANTE y MASAMLOC is suffering from


    cough;

    2. Accused is respectfully requesting for a medical check-up at South


    Cotabato Provincial Hospital, Koronadal City;

    3. Humanitarian justice dictates that the above named accused be


    please given all medical attention to pre-empt any unwelcome
    health consequences;

    4. Attached is the medical referral slip issued by South Cotabato


    Provincial Jail Warden Juan Lanzaderas, Jr., MPA as Annex “A”;
    and

    5. This is in the interest of justice.
