    I, _________________, of legal age, married, and a resident of


    ___________________, after having duly sworn to an oath hereby declare:

    That I am a senior citizen being born on _______and was issued a Senior Citizen
    ID with No. _____ in _____________ dated ____________;

    That last May 13, 2019, I went to ___________________ purposely to


    _________________, and I was bringing with me said Senior Citizen ID Card which I
    placed in my wallet;

    That I even used said Senior Citizen ID Card and presented it to different
    establishments;

    That recently, as I was looking for said ID inside my wallet, I could not anymore
    find it and that despite diligent search and effort to locate my Senior Citizen ID Card, it
    proved futile hence, I hereby declare that the said Senior Citizen ID Card is lost and
    beyond recovery;

    That I am executing this affidavit to attest to the truth of the foregoing facts and
    for whatever legal purposes and intents this may serve.
