    AFFIDAVIT

    I, JHENG A. ZUNIEGA, of legal age, and residing at 7756 Saint


    Paul St. San Antonio Village, Makati City, after having been duly sworn in
    accordance with law, hereby depose and state that:

    1. I am a member of the Social Security System under SSS ID No.


    ________________________;
    2. Said membership was secured on ____________ while I was
    still studying at _______________________;
    3. Lately, my auntie upon learning that I am a member of SSS, helped
    and assisted in paying its monthly contribution.
    4. Right now, I tried to secure my identification card, and it was then
    that my SSS application indicated “for employment”;
    5. To date, I was still unemployed and it was my auntie, who is
    helping and assisting me in the payment of my monthly
    contribution.
    6. That this affidavit is being executed to attest to the truth of the
    foregoing facts and for whatever legal purposes it may served upon
    my person, particularly to correct the information on my SSS
    Membership.

    IN WITNESS WHEREOF, I have hereunto signed this Affidavit this
