1). Looking at this patient’s case, what initial concerns do you have?

 COPD exacerbation, forest fires close to home, oxygen tank safety

nursing process for this patient using ADPIE.
 Assessment- Subjective/objective data collection
 Diagnosis- Impaired gas exchange, risk for ineffective peripheral tissue
perfusion, risk for decreased cardiac tissue perfusion, ineffective self-health
management related to inability to meet increase oxygen demand and dyspnea
on exertion, activity intolerance related to imbalance between oxygen supply and
demand
 Planning- SMART goal development (specific, measurable, attainable,
realisitic/relevant and time restricted
o Avoid irritants, breathing treatment and medication compliance, monitor
O2 sat, encourage healthy weight, encourage movement/activity,
demonstrate pursed lip breathing, notify local fire department and electric
company of tank in use at the home, discuss new living situation
 Implementation- Direct/indirect care to patient
 Evaluation- evaluate if the process is working and identify what is bringing the
individual closer to her goals

5). What evidence do you have to support your chosen nursing diagnosis?
 I have the NANDA to help me formulate the nursing diagnosis. Also Mrs. Grey
has increased her oxygen intake to 5 L and she has obvious signs of increased
work of breathing and anxiety. I know that there was exposure to irritants
continually but did not remove herself from the environment. She was also a
smoker for 48 years and quit 10 years ago
