Please find attached DOST Memorandum from Mr. Teodoro M. Gatchalian, PhD.,
Assistant Secretary for Administration dated 04 April 2019, together with the letter
from Dr. Enrico C. Paringit, PCIEERD,Executive Director, requesting financial
assistance in behalf of MS. JHAZMINE MARISCAL BLAH, daughter of PCIERD
employee, Ms. Myrna M. Blash (SRS I), who underwent a critical operation at
Makati Medical Center to remove a malignant lump in her lung.
Your prayers would be much appreciated and voluntary contribution will be of

great help to settle her medical expenses. Please forward your donations c/o Ms.
Rienzi Joyce C. Jobson, Nurse I, to the Human Resource and Records
Management Section (HRRMS).

Please find attached email from Mr. Thomas Vargas, Representative in Indonesia
of the United Nations High Commissioner for Refugees (UNHCR), a
humanitarian global organization mandated to lead the protection of refugees
and internally displaced communities, dated 8 October 2018, requesting help for
the people who had stricken by earthquake and tsunami in Indonesia. The
organization is continuously accommodating and reaching all of the most
vulnerable, including unaccompanied children, elderly and disabled people.
“A gift of Php 1000 is enough to provide emergency shelter for a person’s monthly
expenses. “
Determined that no one will have to face these disasters alone, your prayers
together with your donations would help meet their most urgent needs. Please

informing us of the demise of MR. EMETERIO Y. CONSTANTINO, father of Ms.
Luciana C. Panaligan, Administrative Officer V, Budget Division, Finance and
Management Service, on 24 September 2018.

from Robert O. Dizon, Executive Director, dated 13 September 2018, requesting
financial assistance in behalf of MR. PABLO M. BALONTONG, SR., father of Mr.
Mr. Pablo M. Balontong, Administrative Aide IV, MIRDC, who was diagnosed with
Hypoxic Ischemic Encephalopathy, Coronary Artery Disease. He is still
unconscious and has been in the Intensive Care Unit (ICU) of the Unihealth
Parañaque Hospital and Medical Center since 2 September 2018.

Director of ITDI and Officer-In-Charge, Office of Director, ALS., dated 18 February
2019, requesting financial assistance in behalf of MR. NOMER A. SANTOS,
brother of Mr. Dunn Alfredo A. Celestial, Administrative Officer V, General Services
Division, Administrative and Legal Service, passed away last February 17, 2019.
Mr Santos was one of the victims in the Edsa shooting incident happened around
3:30 P.M of the same day. His remains lie at Block 5, Lot 17, Scorpio Street,
Veraville Homes 1 Almanza Uno, Las Piñas City.

MEMORANDUM

DATE : 6 March 2019
Director of ITDI and Officer-In-Charge, Office of Director, ALS., dated 27 February
2019, requesting financial assistance in behalf of Mr. Gregorio B. Florendo,
Project Aide from Science Education Institute (SEI), passed away last February
09, 2019 due to Cirrhotic Liver and other abdominal complications. Prior to his
untimely demise, he has been in and out from the hospital and has unpaid
hospital bills. His remains was allowed to be released from the hospital only with
the issuance of promissory note from his family.

Lifeguard Archangels Security Agency Corporation (LASAC), requesting financial
assistance in behalf of MR. ROGELIO A. BUGTO, LASAC Security Guard
assigned at the Packaging Division of DOST-ITDI, who accidentally shot his right
leg with his issued firearm while on duty.

Director of ITDI and Officer-In-Charge, Office of Director, ALS., dated 07 June
2019, informing us of the demise of Ms. Editha A. Manalo, former Supervising
Administrative Officer, Records Section, Department of Science and Technology-
Central Office (DOST-CO), passed away on June 5, 2019 due to Breast Cancer.
Her remains lie in state at Taluto St., corner Anselmo Street, Bauan, Batangas.
Interment is on June 9 at Holy Cross Cemetery, Bauan, Batangas.
