    I, LEANDRO CORONEL, of legal age, Filipino citizen, married to Maria Lourdes V.


    Cortez, with postal address at The Groove by Rockwell, Tower A, Unit 1710A 117 E. Rodriguez
    Jr. Ave. 1604, Pasig City, after having been sworn to in accordance with law, hereby depose and
    say:

    1. That my wife Maria Lourdes V. Cortez and her co-owners of the property located at Brgy.
    Balaytigue, Nasugbu, Batangas. Described in the Transfer Certificate of Title No. T-055-
    2015002048, Prudencio V. Cortez, Jr. and Ethel V. Cortez, are selling the property to
    Shermaine Shaye H. David, of legal age, single, Filipino citizen with residence and postal
    address at Bldg. 26 Unit 31 Guadalupe Mansion, J.P. Rizal Ext., Brgy. Cembo, Makati City
    are herein described as follows;
