Section 1. Registrar’s Office – The Registrar’s Office is ever ready to help students, Faculty members, and the parents who want to avail its services in accordance with its rules and regulations.
1. Request for School Forms (Permanent Record) – (Local and National)
a. No permanent record (SF10) will be released without an official request from school where a student is currently enrolled. b. No permanent record (SF10) will be released unless a student is officially cleared of all school obligations. c. No permanent record (SF10) officially requested by a school with authorization to be hand carried, will be released to the student (owner) personally but to his parents and/or older relatives only. d. No permanent record (SF10) requested for employment purposes shall be released unless equivalent fees are paid to the cashier and official receipt of which presented to the Record Section. e. No permanent record (School Form 10) shall be released for Evaluation Purposes. Only a photocopy will be released, upon request of the school where a student is currently enrolled. After corresponding photo charges are paid. f. CERTIFICATION, AUTHENTIFICATION AND VERIFICATION (DO 48 s. 2017)
2. Request School Forms (Permanent Record)- abroad/overseas in addition to Sec. 1.1 a
and b, the following must be observed: 2.a Equivalent Fees must be paid for the following purposes: 2.a.1 For study abroad- regular rate per page x 2 2.a. 2 For employment abroad- regular rate x 2 per page 2.a.2.1 Request for Diploma 2nd issuance a. No request for a second copy of a school card will be honored unless an affidavit of loss is presented b. An equivalent fee must be paid to the cashier and the receipt together with his clearance form. (No school account status) shall be presented to the record section. c. The person requested may then claim the second copy of the report card the day after.
3. Confidentiality – all school records contained the personal information of the
students are treated confidential in compliance with the Data Privacy Act of 2012
