    AFFIDAVIT OF UNDERTAKING

    I, __________________, of legal age, Filipino, married, and


    a resident of _________________________, under oath, hereby
    depose and state that:

    1. I am the same person executing this Affidavit of


    Undertaking;

    2. I am the owner of a Honda motorcycle, more particularly


    described as:

    FILE NO: ________________

    as evidenced by Certificate of Registration issued by the Land


    Transportation Office and Official Receipt in the name of one
    _________________ which are hereto attached as Annexes “A”
    and “B”;

    3. I acquired the same motorcycle through installment from


    _____________ on November 11, 2017 since it was a repossessed
    property from its previous owner, _____________. Appended
    hereto are copies of the Delivery Receipt, Acknowledgement
    Receipt and a Certificate of Sale issued by the said company as
    Annexes “D”, “D-1” and “D-2”, respectively;

    4. On December 16, 2017, the said motorcycle was involved


    in a motornapping incident which caused the police officers of
    _______________ to take over its custody on 17 December 2017
    pending the investigation thereof;

    5. I earnestly seek to obtain possession of the aforesaid


    motorcycle and custody, I undertake to present and bring it to the
    Honorable City Prosecutor’s custody whenever required to.
