    APPLICATION FOR RELEASE ON RECOGNIZANCE

    The accused, JUAN DELA CRUZ, through the undersigned counsel, hereby
    applies for release on recognizance under the provisions of Section 7, Presidential
    Decree No. 968, as amended RA 10707, and declares:
    1. THAT he filed an application for probation;
    2. THAT, PEDRO dela Cruz, a government employee, a responsible member of
    the community and a resident of xBarangay, xCity, xProvince, is willing to accept
    the custody of the accused and to guarantee his appearance whenever required
    by the court considering that she personally knows the applicant.

    WHEREFORE, above-premised considered, accused hereby prays for his


    temporary liberty upon the recognizance of Pedro dela Cruz pending consideration for
    probation.
