We are pleased to inform you that we are engaging your services as a


____________________ effective _____________ with a (daily/monthly)
rate of __________________________ (P__________). By signing this employment
contract, you agree to engage with your designation and to faithfully abide with the
following terms and conditions of the company:
 

1. You shall be on probation for a period of twelve (12) months reckoning from the
date of appointment until completion of the said period. During your probationary
employment, the Company will determine your fitness to be hired after the
termination of your probationary period. The conversion of your employment to a
permanent status shall be primarily conditioned and shall be dependent upon your
satisfactory service and performance of the work assigned to you. The Company has
the exclusive discretion in determining whether or not such service is satisfactorily
performed and whether or not you have successfully passed and/or complied with the
established standards of the Company for regularization. These standards include,
among others, the following criteria: dependability, trustworthiness, efficiency,
initiative, attitude towards work/ the public/ the Company, its officers and co-
employees, cooperation, customers response, judgment, punctuality, quality/
quantity of work, educability, articulateness and professionalism;

2. The Company likewise reserves its rights to terminate your probationary


employment, even prior to the expiration of your probationary period, for any of the
just and authorized causes subject to procedural due process and contractual due
process as provided by existing laws. In such event, you will be entitled only with the
salary equivalent to your actual rendered service;

3. You shall comply with the all existing rules, regulations and policies of the
Company as well as those which may hereafter be issued, including but not limited to
those governing order and discipline, honesty, safety and security, work assignments
and standard operating procedures, use of Company properties and access to
matters of confidentiality, and such other rules deemed necessary in the
conduct of our business;

4. Probationary employment does not entitle you to the benefits that is or may
hereafter be granted to regular and permanent employees, except those which the
Company as a matter of policy and upon its discretion, extends to all employees
regardless of status and to those provided by law;

5. You agree that all the records and documents of the Company and all
information pertaining to its business and/or its affairs and that of its customers are
absolutely confidential, and any unauthorized disclosure or reproduction of the same
shall not be made by you at any time during or after your employment. Any
breach of confidentiality may cause an immediate termination of your employment
and/or a filing of a civil or criminal action in court as the case may be;
6. You shall be assigned to any work or work station or branch of the Company for
such periods as may be determined by the Company and whenever the service
requires such assignments;

7. You will be liable jointly or severally with other co-workers to any damage you
may cause to any equipment in the place of your designation in case it will be shown
that such damage was caused by your own fault, negligence or abuse of your
position or employment;

8. An amount of P3,000.00 will be deducted from your salary for your board and
lodging and an amount of P1,100.00 for your electric and water consumption;

9. In case you incurred seven (7) days of absences without official leave (AWOL),
you shall be charged with an amount of P 30,000.00 as a penalty.

10. In case you intend to resign from the Company, you shall notify the Company at
least sixty (60) days prior to the effectivity of your resignation except when the
Company accept such resignation earlier than the expiration of said period. Failure to
do so shall render you liable for damages.

11. The imposed penalties in violation of this contract as well as the rules and
regulations of the Company and its Code of Conduct is without prejudice to the right
of Company to file any civil and/or criminal cases in courts regarding the matter in
accordance with the Labor Code, Civil Code and the Revised Penal Code of the
Philippines and any other laws of the law.
