    AFFIDAVIT OF MARITAL STATUS

    That I, ELENA JACELNE DACARA, of legal age, Filipino, with address at Singat, Pura,
    Tarlac, after having been duly sworn to in accordance with law, do hereby depose and state;

    1. That I am the legitimate wife of late ANITO NACIONAL DACARA died last March8 19,
    2015 in Pasay City;

    2. That our marriage was solemnized on August 4, 1999 in Caloocan City

    3. That my husband is a bonafide member of Social Security System;

    4. That the following names are the siblings of my husband ANITO NACIONAL
    DACARA:

     MANUEL NACIONAL DACARA


     ANTONIA NACIONAL DACARA
     PACITA DACARA DUMILAG
     JOSE NACIONAL DACARA

    5. That I being his legitimate wife, I hereby waive all my rights and interest in favor to the
    following names:

     MANUEL NACIONAL DACARA


     ANTONIA NACIONAL DACARA
     PACITA DACARA DUMILAG
     JOSE NACIONAL DACARA

    6. That I, decided to waive all my rights and interest to his siblings because ANITO and I
    been separated for several years and we have no child;

    7. That I am allowing Social Security System to transfer all my claims to them;

    Further, this affidavit is made and executed in order to explain the marital status of the
    deceased and for whatever legal intents and purposes this affidavit to be used, and serve.
