    REQUIREMENTS FOR JAPAN VISA:

    1. 1 pc picture 2X2 colored with white background, wear with collar, no eyeglasses
    (45mm x 45mm).
    2. Xerox copy of Income Tax Return (A4 SIZE PAPER)
    3. Xerox copy of Business Name or Original Employment Certificate. (A4 SIZE
    PAPER)
    4. Original Bank Certificate and Xerox copy of bank cert on A4 size paper.
    5. Original NSO Birth Certificate and Marriage Certificate (not more than 6 months)
    6. Old and New Passports.
    7. Copy of Daily Itinerary (A4 SIZE PAPER)

    STUDENTS:
    1. Xerox copy of School I.D. (A4 SIZE PAPER)
    2. Original affidavit of support. (notarized) if parents will not accompany the child
    3. Original NSO Birth Certificate (not more than 6 months).

    If with previous Japan Visa:


    1. Attach old passport with the Japan visa.
    2. All requirements are the same as stated above.
    except: only xerox copy of Birth Certificate and Marriage Contact are needed. (no need
    for NSO copy).

    If Birth Certificate is Late Registry


    - additional original baptismal certificate
    - original transcript (school form 137)

    Housewife:
    - all requirements as stated above
    - affidavit of support from husband (if husband will not accompany the wife)

    Chinese Citizen:
    - I-Card Xerox copy back to back (A4 SIZE PAPER)

    Senior Citizen:
    - Xerox copy of senior ID back to back (A4 SIZE PAPER)

    Philippine Passport – PHP 600 (If joining tour our)


    Philippines Passport – PHP 800 (If buying hotel or ticket)
    Philippine Passport – PHP 1,000 (Visa only; no other services)
    Taiwan and PROC Passport – PHP 2,100 (If joining our tour)
    Taiwan and PROC Passport – PHP 2,300 (If buying hotel or ticket)
    Taiwan and PROC Passport – PHP 2,500 (Visa only; no other services)
