    AFFIDAVIT/ADDENDUM

    I, JUANITO A. FLAVIER, of legal age, Filipino, and with address at 628 Hillside, Baguio City
    2600, after having been duly sworn to in accordance with law, depose and state:

    1. This is to certify that a Deed of Transfer of 7749 SMDC share by JUAN FLAVIER in favor of
    JUANITO FLAVIER was made in August 20, 2014 and duly notarized by Atty. Roy Carlos with
    Doc. No. 21 Page No. 5, Book No. 11 and Series of 2014;

    2. That an erasure was made to adjust correct number of shares at 7,749 SMDC shares. That the said
    erasure was duly noted by the notary public ATTY. ROY CARLOS;

    3. That I am executing this affidavit in order to attest the truth of the foregoing facts and for
    all legal intents it may serve

    IN WITNESS WHEREOF, I have hereunto set my hand on this 17 th day of December 2014 at
    Manila, Philippines.
