    AFFIDAVIT OF LOSS

    I, GREGGY ALFARERO, Filipino, of legal age, married and a resident of


    Medellin, Cebu, Philippines, after being duly sworn in accordance with law, depose
    and state:

    1. That I have travelled from Tacloban to Cebu via Cebu Pacific;


    2. That I was issued a boarding pass at Daniel Z. Romualdez Airport ;
    3. That while in my possession the said boarding pass was misplaced and got
    lost.

    4. That despite diligent search and efforts to locate the said boarding pass, I
    could not find the same such that I now believe that it is now lost beyond
    recovery.
    5. As such, I am executing this Affidavit of Loss to attest to the truth of the
    foregoing facts.

    IN WITNESS WHEREOF, I have hereunto set my hand this _____________ at
