PETITION
Petitioner xxx, respectfully states:

1. The petitioner is a Filipino, single, of legal age, having


been born on xxx, and residing at xxx.

2. She obtained her law degree in 2015 and admitted to the


practice of law in 2016. She is a member in good standing of the
Philippine Bar, as certified by the Office of the Bar Confidant of the
Supreme Court1 and the Integrated Bar of the Philippines. She is also
a person of good moral character.2

3. She holds office at the xxx with cellular phone nos. xxx.
For 2016, Atty. xxx has dutifully paid the taxes due from her as such
professional.3

4. She has not been convicted in the first instance of any


crime involving moral turpitude or of any crime of whatever kind or
nature.

5. She seeks of this Honorable Court her appointment as a


Notary Public for and in the City of Davao, and the issuance of a
notarial commission in her favor.

She likewise seeks of this Honorable Court the approval of her
Notarial Seal,4 which is made of metal, circular in shape, two inches
in diameter, and bears her name “xxx”, the words “Davao” and
“Philippines” on the margin, her Roll of Attorney’s number, which
is xxx, with the words “NOTARY PUBLIC” across the center, which
thus complies with Section 2 (a), Rule VII of the 2004 RULES ON
NOTARIAL PRACTICE (A.M. No.-02-8-13 SC) which provides:

“SECTION 2. Official Seal. - (a) Every person


commissioned as notary public shall have a seal of office, to be
procured at her own expense, which shall not be possessed or
owned by any other person. It shall be of metal, circular in shape,
two inches in diameter, and shall have the name of the city or
province and the word “Philippines” and her own name on the
margin and the roll of attorney's number on the face thereof, with
the words "notary public" across the center. A mark, image or
impression of such seal shall be made directly on the paper or
parchment on which the writing appears.”

WHEREFORE, the petitioner respectfully prays for her


appointment as Notary Public for and in the City of Davao,
Philippines, and for the approval of her Notarial Seal.
