    UNDERTAKING

    I Dr. Rachana Vishwajeet, previously known as Dr. Rachana Chandrakant Ingle Reg. no. 2009041711
    residing at 58/2640, Ravikiran CHS, Gandhi nagar, Bandra East, Mumbai 400051 gives undertaking to
    the Maharashtra Medical Council, Mumbai as under :-

    That,

    1) The CME (Continuous Medical Education) certificate/s and the credit points by me today for
    renewal of registration are true and correct.
    2) I further undertake that I will collect the required CME points which are less than required 30
    points to be collected in 5 years within next year. I will submit the certificates of these credit
    points.
    3) If at any stage, the council has noticed or observed that, the CME certificate/s has been
    produced by me is/are false, fabricated, bogus and fraudulent, then the Council has liberty to
    pass any order of punishment in view of Section 22 of Maharashtra Medical Council Act, 1965
    and I am also aware that, I shall be liable for the prosecution under Indian penal code for the
    same.

    I undertake that I shall regularly attend CME programme in future as required for my Professional
    update.

    I therefore, request Maharashtra Medical Council to renew my registration.
