Nursing Care plan for Internal

Nursing diagnosis (1):
 Pain related to fracture, soft tissue damage, muscle spasm,
and surgery evidence by patient facial expression or pain
scale.

Goals :
 Relieve patient pain during 24 hours & maintain patient
comfort.

Nursing interventions :
 Assess patient pain after surgery by using pain scale
 Assess patient vital signs specially pulse & BP.
 Adjust patient position of site of fixation (by elevation the
limb).
 Instruct & teach patient about relaxation technique
 Administer analgesic as order

Evaluation :
 Goals was met during 24 hours evidence by patient free
from pain, relax and sleepy.

Nursing diagnosis (2) :
 Impaired physical mobility related to fractured hip
evidence by patient not able to move.

Goal :
 Maintain patient physical mobility, prevent muscle
atrophy and bed sores during month of treatment &
staying in hospital.

Nursing interventions :
 Assess patient physical mobility & level of dependency
 Assess muscle tone, mass & flacity.
 Advice patient to use moving aid device in movement
 Turning & lifting patient every 2 hours.
 Apply passive range of motion exercise every day.
 Apply muscle massage after bed bath every day

Evaluation :
 Goal wase met & achieved through outpatient stay in
hospital evidence by absent of immobility complications.

Nursing diagnosis (3) :
 Impaired skin integrity related to surgical incision
evidence by surgical site wound & special screws, plates,
rods, wires.

Goal :
 Maintain normal skin integrity & speed healing of wound
through out month period.

Nursing intervention :
 Assess skin integrity at site of incision
 Assess healing process every day during dressing
 Assess peripheral pulse and sensation of affected limb
 Apply wound care every day
 Encourage patient to eat protein and vitamin C

Evaluation :
 Goal was met & achieved evidence by intact skin integrity
& wound healed properly throughout month period .

Nursing diagnosis (4) :
 Risk for infection related to fracture and surgery

Goal :
 Prevent patient infection throughout course of treatment

Nursing interventions :
 Assess patient signs of infection by measuring TEMP daily
and assess the site of wound for redness, swelling &
tenderness.
 Applying dressing daily
 Administer antibiotics as order

Evaluation :
 Goal was met & achieved evidence by patient free from
infection, temp stable and normal.
