    AFFIDAVIT OF MARITAL CONSENT


                  I, ____________, of legal age, Filipino, married to _______________,
    with residence and postal address at _______________, Philippines, having
    been duly sworn in accordance with law, hereby depose and state that:
             
    1. My spouse and I have agreed and have decided to apply for an auto
    loan with Banking Corporation to finance the purchase of a motor vehicle from
    Ford Dumaguete, for the benefit of our family;

    2. As part of Banking Corporation documentation requirements for the


    auto loan, my spouse and I are being required by Banking Corporation to sign
    the requisite loan and mortgage documents and other related documents as
    borrowers and mortgagors;

    3. My spouse cannot sign any and all such documents because he is


    currently out of the country. He is out of the country working as a civil engineer in
    Doha, Qatar;

    4. My spouse has consented to and has given his permission to apply


    for such auto loan, get the loan proceeds to purchase the Vehicle for the benefit
    of our family, secure the loan with a chattel mortgage on the Vehicle, and sign
    and execute any and all documents for the loan and chattel mortgage;

    5. Attached hereto and made an integral part hereof as Annex A is a


    photocopy of our marriage contract. Likewise attached hereto and made an
    integral part hereof as Annex B is the proof that my spouse is employed as an
    OFW at ___________________;

    6. I am executing this affidavit to attest to the truth and correctness of


    the foregoing statements.
     
    IN WITNESS WHEREOF, I have hereunto set my hand this 14th day of
    February 2017 at Dumaguete City, Negros Oriental, Philippines.
