    JUDICIAL AFFIDAVIT

    I, MARIA ANTONIETA LABO, of legal age, single, Filipino, and


    a resident of Room 1, Dimakita Bldg., Makawala St., Davao
    City, Philippines, having been duly sworn in accordance with
    law, hereby depose and state that:

    1. I was examined by Atty. Matalas A. Isip of the ISIP Law


    Firm located at 0013 Rizal Ave., Davao City,
    Philippines;

    2. I answered the questions asked of me, fully conscious


    that I do so under oath, and that I may face criminal
    liability for false testimony or perjury.

    The following are the questions asked of me by Atty. Isip,


    and my corresponding answers:

    Q1. Please state your name and other personal


    circumstances for the record.

    A1. I am Maria Antonieta Labo, Filipino and residing in


    Davao City
    Q2. Are you the same Maria Antonieta Labo, the
    petitioner in this case?

    A2. Yes.

    Q3. Do you know a certain Pablo Dipakau Halap?

    A3: Yes. He borrowed money from me in the sum of Php


    500,000.00

    Q4: When did Pablo borrow money from you?

    A4: November 20, 2017

    Q5: Do you have any proof that Pablo borrowed money


    from you on the said date?

    A5: Yes. He executed a promissory note dated


    November 20, 2017 where he had acknowledged his
    indebtedness and where he promised to pay the sum of PHP
    500,0000 on January 20, 2018, (ANNEX “A”)

    Q6: Did Pablo pay his indebtedness on January 20,


    2018?

    A6: He failed to pay,

    Q7: How did you settle this matter?

    A7: Repeated demands both written and verbal were


    sent to demand payment of the indebtedness. (ANNEX “B”
    to “B1”)

    Q8: Did he answer your demand?

    A8: He failed and refused to comply with the obligation.
