    DEED OF DONATION

    KNOW ALL MEN BY THESE PRESENTS

    This DEED OF DONATION, made and entered into by and between:

    ______( name of donor )________ , an organization registered under the laws of


    the Republic of the Philippines, (skip this portion if the donor is a natural person/not a
    company and go directly to the address of the donor.) with address at
    ____________________________, hereiafter referred to as the “DONOR”.

    Police, an agency created and existing by virtue and pursuant to R.A. 6975 as
    amended, represented herein by _________________________, hereinafter referred to
    as the “DONEE”.
    WITNESSETH:
