    EXTRA-JUDICIAL SETTLEMENT
    OF THE ESTATE OF THE DECEASED

    KNOW ALL MEN BY THESE PRESENTS:

    WHEREAS, NAME died intestate on DATE, as evidenced by a copy of his


    Certificate of Death, hereto attached as Annex “A” of this Extra-Judicial Settlement;

    WHEREAS, the residence of the deceased at the time of his death is ADDRESS;

    WHEREAS, the deceased NAME is survived by his spouse, NAME, and the
    following legitimate children as his legal heirs, namely:

    1. NAME, married to NAME, both of legal age, Filipino citizens and


    residing at ADDRESS;

    2. NAME, of legal age, single, Filipino citizen and residing at ADDRESS;

    WHEREAS, the said deceased NAME, is the true, lawful, and absolute owner

    The copies of the Transfer Certificates of Title and Original Certificate of Title are
    hereto attached and made an integral part hereof as Annexes “B’, “C” and “D”
    respectively;

    WHEREAS to the best knowledge and information of the Heirs, there are no
    debts or obligations due against the Estate of the deceased NAME;

    WHEREAS, aside from the above-named legal heirs, the said deceased has no
    other children (legitimate or otherwise);

    WHEREAS, the above-named legal heirs have agreed to settle extra-judicially the
    share of the deceased NAME in the above described properties ;

    NOW, THEREFORE, the foregoing premises considered, the above-named legal


    heirs, by these presents, do hereby agree to divide and adjudicate, as they hereby divide
    and adjudicate among themselves in pro indiviso equal shares the Estate of the deceased
    NAME;

    The parties hereby affirm that they have executed the foregoing instrument out of
    their own voluntary free will, without force, intimidation or violence upon their person.
