1. Accomplished cash assistance application form to be filled-out in this system


2. Scanned copy of 2 valid IDs of employee, preferably Office, Company and SSS/GSIS or
other Government ID (for Domestic Employees only)
3. Certificate of Employment from Employer including a declaration of the member's last
day of work prior to contracting the illness due to Covid-19 (for Domestic Employees
only)

4. Medical Abstract/Medical Certificate for COVID-19 illness or death
5. Laboratory test result (RT-PCR) showing POSITIVE for COVID-19 issued by a DOH
accredited testing center (for Domestic Employees and Overseas Seafarers who tested
COVID-19 Positive during their quarantine period in the Philippines upon repatriation
from abroad)
6. Copy of Seafarer's Identification Document (SID) or ID and signature pages of passport;
and passport page showing stamp of arrival in the Phils. (for Overseas Seafarers)
7. Copy of Overseas Employment Certificate (OEC) covering overseas employment during
pandemic and repatriation period because of COVID-19 (for overseas seafarers)

1. Death Certificate
2. Marriage Certificate (for claimant spouse)
3. Birth Certificate of child (for claimant child)
4. Any 2 Valid IDs of claimant
5. Affidavit of guardianship (if applicable)

For Unmarried Employees

1. Death Certificate
2. Birth Certificate of employee (for claimant parent)
3. Any 2 Valid IDs of claimant
4. Affidavit of guardianship with Birth Certificate of dependent child of the deceased
employee (if applicable)
