AFFIDAVIT

I, ROBERTO I. CRUZ, of legal age, Filipino, married, and with postal


address at Brgy. Pagsawitan, Santa Cruz, Laguna, after having been sworn
in accordance with law, hereby depose and state that –

1. I am a geodetic engineer by profession. As such, on 10 December


2015, I undertook the consolidation and subdivision survey of Lot
No. 3246-F-10-A covered by Transfer Certificate of Title No. T-
205199 with an area of 443 square meters; and Lot No. 3246-G
covered by Transfer Certificate of Title No. T-115315 with an area of
313 square meters. Said two lots were consolidated and subdivided
resulting into five (5) lots, eventually designated as Lot Nos. 1, 2, 3,
4, and 5, PCS-04-030255;

2. The said consolidation/subdivision survey was undertaken by me for


and on behalf of Spouses Roman Camarillo and Rufina Camarillo;

3. Among the lots formed by virtue of the said survey are as follows –

 Lot 2 PCS-04-030255 covered by TRANSFER CERTIFICATE OF


TITLE NO. 058-2018004321 and described as:
“Lot No. PCS-04-030255, being a portion of Lot 3246-G, PSD-04-
0301 & Lot 3246-f-10-A PSD-04-122611, located at Brgy. San
Juan, Mun. of Santa Cruz, Province Laguna;

which document has been previously entered in the Notarial Register
of Notary Public Atty. Emmanuel E. Sandicho on 18 February 2016 at
Doc. No. 109; Page No. 23; Book No. II; Series of 2016. And as such,
for all intents and purposes, covered and bound by such legal
disposition.

TITLE NO. 058-2018004323 and described as:

February 2016 at Doc. No. 93; Page No. 20; Book No. II; Series of
2016. And as such, for all intents and purposes, covered and bound
by such legal disposition.

4. I executed this affidavit to attest to the truth of the foregoing statements


and for all and any other lawful purposes this may serve.
