JUDICIAL AFFIDAVIT OF ENGR. RAMON S. UNAY

I, Engr. Ramon S. Unay, 52 years old, married, with office address at
the DENR Regional Office No. VIII, Tacloban City, after being duly sworn
to in accordance with law, hereby depose and state:
PRELIMINARY STATEMENT
This Judicial Affidavit of Engr. Ramon S. Unay was taken and
recorded based on the examination made and conducted by Atty. Marie
Ronette Salve E. Barreda of the City Government of Tacloban with business
and office address at City Legal Office, Kanhuraw Building, Magsaysay
Boulevard, Tacloban City.
I hereby attest that at the time of examination, I was fully aware and
conscious that I did so under oath and that I may face criminal liability for
false testimony or perjury. I answered the questions propounded to me based
on my personal knowledge and authentic document/s with full knowledge
and understanding on every question asked.
This Judicial Affidavit is to be submitted to the Regional Trial Court,

testimony in relation to Cadastral Case NO. R-TAC-16-01177-LR entitled
City Government of Tacloban vs The Register Deeds of Tacloban City, for
Reconstitution.
EXAMINATION PROPER
1. QUESTION: Please state your name and other personal
circumstances.
ANSWER: I am Engr. Ramon S. Unay, 52 years old, married, a

resident of Brgy. 95, Caibaan Tacloban City and presently employed as
Chief of the Survey and Mapping Division, DENR Regional Office No.
VIII, Tacloban City.
2. Q: What is your purpose in executing this Judicial Affidavit?
A: I am executing this Judicial Affidavit to prove the following:
1. That I am the Chief of the Survey and Mapping Division of

the DENR Regional Office VIII;
2. That our office issued a Technical Description covering Lot

No. 4430 based on the Lot Data Computation available in our
record files;
3. That based on the Lot Data Computation, Lot 4430 located at

Barangay Cawayan, Tacloban City, was surveyed on January
19, 1927 by Engr. F. Agustin and such survey was approved in
April 1928;
4. To confirm the details set forth in the Technical Description;
5. To identify relevant documents and testify on other matters.
3. Q: You mentioned that you are the Chief of the Survey and Mapping
Division, DENR Regional Office No. VIII, what are your duties as such?
A: As Chief of the Survey and Mapping Division, I am in charge of

reviewing, signing and issuing Technical Descriptions of lots and other
documents in our office, among others.
4. Q: Since when have you been on this job as a Chief of the Survey and
Mapping Division?
A: I have been in this job for sixteen (16) years.
5. Q: Mr. Witness, being the Chief of the Survey and Mapping Division,
do you remember issuing a Technical Description dated 2 August 2017 for
Lot No. 4430 located at Brgy. Cawayan, Tacloban City?
A: Yes maam. We issued a Technical Description for Lot 4430 last
August 2, 2017.
6. Q: Can you please tell this Honorable Court what this document is all
about?
A: The Technical Description that we issued is a document that tells

the metes and bounds of the lot including the coordinates, the bearing and
distances of the boundary lines, sketch and other pertinent information about
Lot 4430.
7. Q: At the bottom of this document, are the words “Certified Correct”

and below it is your name with a signature. Is this your signature?
A: Yes maam, it is my signature.
ATTY. BARREDA: I am now attaching a copy of the Technical Description

of Lot No. 4430 issued by the DENR Regional Office VIII as Exhibit “O”
for plaintiff City.
8. Q: Do you confirm my action Mr. Witness?
A: Yes maam.
9. Q: Mr. Witness what then was the basis of our office in preparing the
Technical Description for Lot 4430?
A: The Technical Description is based on the Lot Data Computation

on file in the office.
10. Q: You mentioned the Lot Data Computation, I have here a copy of
the Lot Data Computation for Lot No. 4430 located at Barangay Cawayan,
Tacloban City. Is this the one you are referring to?
A: Yes maam, this is the Lot Data Computation.
11. Q: Do you want this Lot Data Computation attached to your Judicial
Affidavit?
A: Yes maam.
ATTY. BARREDA: I am now attaching a copy of this Certified Lot Data
Computation marked as Exhibit “P” for plaintiff City.

A: It is located at Brgy. Cawayan, Tacloban City.
16. Q: What is the area of the lot?
A: According to the Lot Data Computation, the area is 24, 974 square
meters.
17. Q: Do you know if who requested for the survey of Lot 4430?
