    The Office of the Provincial Prosecutor of Antique (OPP-


    Antique), by undersigned Prosecution Attorney and unto this
    Honorable Court most respectfully states:

    1. The Pre trial Conference was conducted on __

    2. That the Pre trial order states only __ is to be presented by


    the Prosecution:

    3. However in the interest of justice, the prosecution prays


    that the Pre-trial order be amended to include the doctor as additional
    witness to be presented during trial;

    4. The doctor will testify on his findings and the latter was not
    named during the Pre-trial conference:

    5. That this motion is not intended for delay or pro long the
    proceeding in this case.

    WHEREFORE, it is prayed of this Honorable Court that the Pre-


    trial be amended to include the doctor as witness to be presented
    during trial.

    The prosecution further prays for other just and equitable relief
    under the premises.
