    AFFIDAVIT OF RESIDENCY

    I, MARK HODGSON, a British citizen, of legal age, married and a


    resident of Purok 2, Maular, Aloran, Misamis Occidental, Philippines under
    oath, hereby depose and say that:

    1. I am married to Maquelo Baring-Hodgson, a Filipino citizen;

    2. I and my wife are currently living in Purok 2, Maular, Aloran, Misamis


    Occidental, Philippines for six (6) years already as evidenced by his electric
    consumption bill issued by Misamis Occidental I Electric Cooperative Inc.
    (MOELCI I) Calamba, Misamis Occidental, Philippines marked as Annex A;

    3. I possess an identification cards issued by the Republic of the


    Philippines such as Alien Certificate of Registration issued by the Bureau of
    Immigration of the Republic of the Philippines and Non-Professional Driver’s
    License issued by the Land Transportation Office of the Department of
    Transportation, Republic of the Philippines and are attached hereof and mark
    as Annex B;
    4. I cause the preparation of this affidavit to support the cash-in policy of
    my PRUDENTIAL PLAN.
