    ages, Filipino citizens, and with residence at __________________________, Philippines, do hereby
    NAME, APPOINT and CONSTITUTE, ATTY. _______________________ and/or MR.

    ________________ issued by the Registry of Deeds for Calamba (”REAL PROPERTY”);
    3. To request for and obtain copies of the latest the tax declaration of the property and other
    documents and certificates from the proper City Local Government Office;
    4. To accomplish pertinent Bureau of Internal Revenue (“BIR”) forms based on the ONETT
    computation sheets for the Estate Tax, Documentary Stamp Tax (“DST”) and Capital
    Gains Tax (“CGT”), if applicable;
    5. To file the Estate Tax Return, DST return and CGT return and pay the pertinent taxes at
    the Authorized Agent Bank of the Revenue District Office;
    6. To obtain Certificate Authorizing Registration (“CAR”) from the BIR;
    7. To pay the real property taxes and obtain tax clearance certificate of real property taxes
    from the City Local Government Office;
    8. To apply for the issuance of a new tax declaration over the property in our name;
    9. To apply with the proper Register of Deeds the registration and issuance of new title in
    our name;
    10. To apply with the proper Local Civil Registrars all pertinent Certificates of Birth,
    Marriage Certificates, etc. related to the processing of the above mentioned transactions;
    11. To pay the required fees and costs and receive the official receipts;
    12. To perform all acts as may be necessary or proper to fully and effectively implement the
    foregoing.

    HEREBY GIVING AND GRANTING unto said Attorneys-in-Fact full power and authority
    to do and perform all and every act and thing whatsoever requisite and necessary to be done in and
    about the premises, hereby ratifying and confirming all that said Attorneys-in-Fact shall lawfully do
    or cause to be done by virtue of these presents.
