This document contains a static and dynamic postural assessment form. The static assessment examines the subject's posture from the front, back, and sides and looks for signs of pes planus, lower crossed syndrome, or upper crossed syndrome. The dynamic assessment evaluates the subject's overhead squat, single-leg squat, and pushing/pulling motions and identifies any impairments in movement. It also lists which muscles may be overactive or underactive based on the observed impairments.
