Procedural Checklist in
Colostomy Irrigating and Appliance Care.

General Objective:
Given simulated situations/conditions, the students will be able to demonstrate the proper procedure in
Colostomy Irrigating and Appliance Care.

Specific Objectives:
1. To empty the large colon of the stool and fecal materials.
2. To distend the bowel sufficiency to stimulate peristalsis which stimulates evacuation.

1. Wash hands and don gloves and      
Close room curtains or door.
2. Prepare the patient and the      
equipment:
 Explain the procedure to the
patient.
 Hang irrigator with 500-1000 of
water about 12-18 inches high.
Prime tubing and then clamp
irrigating tube.
 Put Kelly pad alongside colostomy
bag.
 Place patient in side-lying position.
 Make a hole in the colostomy
appliance.
3. Lubricate tip of your little finger then      
insert it into the stoma followed by
the lubricated cone, insert cone 2-4
inches deep into the stoma.
4. Attach irrigating tube to the cone.      
5. Make a small cut in to colostomy      
bag.
6. Open clamp and irrigate stoma for      
10-15 minutes, close. After 10-15
minutes, close clamp and remove

7. Remove old appliance gently as      
one piece.
8. Wash skin gently with skin cleanser      
or with regular soap and water.
Remove from the skin. Wipe with
gauze or washcloth and rinse soap
thoroughly. Blot dry.
*If blood appears when washing,
reassure the client that small amount
is normal. Continues bleeding is
abnormal after pouch is in place
“refer to health care provider.”
9. 9. Observe condition of skin,      
stoma, sutures; encourage patient
to make these observations at time
of pouch changes or if skin feels
irritated. Skin must be absolutely
dry (paper towel) and Apply (karaya
powder per agency and/or per
doctors order).
10. 10. Measure the size of the stoma      
and cut the center of the new
pouching system approximately
atleast 2 cm wide on the actual size
of the stoma.
11. Have pouch ready by removing      
backing from barrier. Apply the
pouch to skin after cleaning and
drying skin. Smooth out from
center. Hold in place firmly for 1-3
minutes. Apply position that
facilitates emptying.
12. Apply hypoallergenic tape as      
needed to edges of faceplate over
skin barrier. Some manufacturer
pouches are with tape in place.

13. Fold bottom edges of pouch over to      
fit over clamp. Secure clamp.
14. Dispose old appliance in plastic      
bag or trash can. Remove gloves
and do “Handwashing.”
15. Document the procedure and
