    REGIONAL TRIAL COURT OF DAGUPAN CITY, PANGASINAN

    The undersigned, ARAMINA, hereby applies (or apply) to have the


    land hereinafter described brought under the operation of the Property
    Registration Decree, and to have the title thereto registered and confirmed:
    AND DECLARE . . . . .
    1. That the applicant is the owner of the land (by virtue of
    inheritance), together with the building and improvements thereon.
    The said land, consisting of 1,000 square meters parcels are
    situated, bounded and described as shown on the plan and
    technical descriptions attached hereto and made a part hereof;
    2. That said land at the last assessment for taxation was assessed at
    P 2,030.00, Philippine currency, and the buildings and other
    improvements at P 4,700.60, Philippine currency;
    3. That to the best of my knowledge and belief, there is no mortgage
    or encumbrance of any kind whatsoever affecting said land, nor
    any other person having interest therein, legal or equitable, or in
    possession;
    4. That the applicant has acquired said land in the following manner:
    inheritance from predecessor-in-interest on which has been
    occupying the subject land for 80 years starting from 1940 up to
    present;
    5. That said land is solely occupied by the applicant;
    6. That the name in full and address of the owner of the property are
    as follows: ARAMINA, Municipality of San Fabian, Province of
    Pangasinan;
    7. That the applicant is single;
    8. That the applicant’s full name, age, citizenship, and residence is as
    follows: Aramina, 22 y/o, Filipino, Municipality of San Fabian,
    Province of Pangasinan; and
    9. That the following documents are attached hereto and made a part
    hereof:
    a. Plan in tracing cloth of land attested and duly certified by
    the Regional Technical Director;
    b. Blue print copies of the plan;
    c. Technical descriptions certified by the Regional
    Technical Director;
    d. Geodetic Engineer’s certificate; and
    e. Certificate of the Municipal Assessor of the assessed
    value of the land at its last assessment for taxation.
    Signed at Municipality San Fabian, Province of Pangasinan this 31 st
    day of October, in the year two thousand and twenty.

    that the statements therein are true of his knowledge, information and belief.
    The Community Tax Certificate of the applicant was exhibited to me
    being No. 0000010220 issued at Municipality of San Fabian, Pangasinan
    dated June 1, 2020.
