'I acknowledge that I will inform my manning agent immediately if, in the 14 days before my travel date:
I am in close contact to anyone diagnosed with or under investigation for COVID-19

- High temperature
- A loss of, or change to, your sense of smell or taste.

PCR testing
We are closely monitoring the evolution of the Covid pandemic and as an additional measure to minimise the
risk of introducing this illness on board our ships, we are asking that all crew members undertake Covid19 RT-
PCR testing prior to departing from their home to join the ship.

Non-UK Residents joining the ship in UK Ports:

The test must be conducted not less than 24 hours and not more the 72 hours prior to your departure to the
UK. Please consider that results are usually issued approximately 24 - 36 hours after the sample reaches the
lab. PCR proof may be requested at airport check-in, please carry this documentation with you. Please do not
delay in making arrangements for your test, as while the test cannot be done more than 72 hours prior to your
departure to the UK, there is a high demand for the RT-PCR Testing and so appointments need to be made in
advance.

UK Residents joining the ship in UK & Non-UK Ports & Non-UK Residents joining in Non-UK Ports:
The test must be conducted not less than 24 hours and not more the 5 days prior to your Joining date. Please
consider that results are usually issued approximately 24 - 36 hours after the sample reaches the lab. Where
possible, testing should be undertaken in the 72 hours prior to leaving home to travel to the ship. However, if
you are using a postal testing option and delivery of the sample will be impacted by no post on Sundays, or,
the 72 hours window cannot be met due to weekend Lab closure, a test taken up to and no more than 5 days
before your joining date will be accepted. The test must be conducted by an accredited laboratory.

COVID-19 Vaccination Status
We fully support this effort and strongly encourage you to get vaccinated in your home country as soon as it is your
turn. Successful COVID-19 vaccination programmes are a major step in resuming normal cruise operations. Being
vaccinated protects our team members, our Guests, and the communities we visit.
Carnival UK is working hard to support home nation and shipboard vaccination. To help us with scheduling and
planning of vaccination programs, we will need information about your current vaccination status. Vaccination
status means either being fully vaccinated, partially vaccinated, or not vaccinated.
Knowing everyone’s status will help us determine vaccination needs for our team on board, and at home. We may
also need to share your vaccination status with port and public health authorities. Information you share about your
vaccination status will be stored as part of your medical record.
As the COVID-19 pandemic evolves, our vaccine protocols may change. We will keep you informed of any changes
and new requirements, through direct communications and privacy notices.

If you have not been vaccinated yet, or if you get vaccinated before your next assignment:
Please submit information on your vaccination status, via this safe and secure link:
https://www.cognitoforms.com/CSSIPvtLtd/COVID19VaccinationStatus

You do not need to submit your details if:

You previously provided your vaccination data to our shoreside medical team (via Covid Query) or to our CUK
Onboarding team.
You previously provided your vaccine information to the medical staff on board.
You have been vaccinated while on board one of our ships.
You were vaccinated at a company arranged vaccination clinic ashore or on board.

To help you make informed decisions about your health, we recommend that you review science-based information
published by health authorities. For key facts and safety information, please review the following information
