    Affidavit of undertaking

    I, ____________________, of legal age, Filipino, married, and a resident


    of Barangay Tayud, Consolacion, Cebu, after having been sworn to in
    accordance with law, hereby depose and say:

    (1) That I am a barangay councilor of Barangay Tayud, Conslacion, Cebu;

    (2) That I personally know ______________________ to be a resident of our


    barangay;

    (3) That I understand that after conviction, the convicted accused applied for
    probation in his/her criminal cases for violations of Sections 5 and 11 of
    Republic Act No. 9165, before the Regional Trial Court-Branch 88,
    Mandaue City;

    (4) That I am ready and willing to assume and vouch for the custody of the
    convicted accused if released during the pendency of the resolution of
    his/her application for probation;

    (5) That I am likewise solemnly committing and undertaking that I will


    guarantee and be personally liable for the appearance of the convict when
    required by the Honorable Court or the Parole and Probation
    Administration office during the abovementioned period;

    (6) That should the convict’s application for probation be denied, I will assist
    the Honorable Court for his/her arrest.
