4. Accuracy

Performs dose calculations accurately. Partially Meets Expectations [5 pts]


Additional Comment: Muath continues to
make many repeat errors. Example:
Muath worked on a scalp plan on 7/29.
He did not create a plan sum with
previous RT that was 3-4cm inferior. The
CMD explained the importance of
creating plan sums even if there is not
direct overlap. The next day he
attempted a comp on a plan with
overlapping fields from previous RT. He
did not create a plan sum and was failed.
On 8/10 he again did not create a plan
sum with previous RT 1-2cm away which
was caught by the on site CMD. He was
so skeptical of the necessity of a plan
sum that he called me to discuss why it
needed to be created. I encourage
questions, of course, but this is the 4th or
5th time the importance of plan sums
and when to create them has been
discussed/explained. This is a concerning
trend.
Reads radiation measurement devices
accurately and carefully. (eg, TLD, Diodes, N/A
Film, Ion Chamber)
Accurately designs plans by computer or Partially Meets Expectations
manual computation in accordance with Additional Comment: Muath's field in
prescription. field and 3D plans are consistently good.
He is not, however, where he should be [5 pts]
at this point in the program. Straight
forward 3-field breasts and simple VMAT
plans should be relatively easy at this
point.
Accurately enters information into chart Partially Meets Expectations
or R & V system. Additional Comment: Muath knows the [5 pts]
documentation that is needed but is not
consistently doing this without error.

biggest problem area. He struggles to
digest information that he has been
given in the past and apply it to the
current patients he is working on. He

use for the job.

It is my opinion that Muath needs to over prepare in order to be successful. If he knew the fundamentals
from his course work inside and out, he could apply that to the clinic and still be successful. It appears that
he has trouble retaining the knowledge necessary to competently produce high quality treatment plans.
