CONTRACT OF SERVICES
KNOW ALL MEN BY THESE PRESENTS:
This Contract, made and executed in the City of Zamboanga, Philippines, by and
between –
JOSELITO , of legal age, single, Filipino and a resident of _________________,

Zamboanga City, hereinafter referred to as the FIRST PARTY;
-AND-
(employee’s name) , of legal age, (status) Filipino and a resident of

________________, Zamboanga City, hereinafter referred to as the SECOND PARTY;
WITNESSETH;
That the First Party and the Second Party, in consideration of the mutual covenants and
agreements made herein, by the one to the other, by these presents enter into a contract of
services, whereby the First Party hires and employs the Second Party under the following terms
and conditions:
1. The First Party shall employ the Second Party for a term of ______ years
from the date hereof as the Assistant of the First Party’s business called
__________ , now carried on at (business address) ;
2. The compensation or salary to be paid by the First Party to the Second Party
for the services to be rendered by the latter to the former shall be FOUR
THOUSAND FIVE HUNDRED PESOS (Php 4,500.00) Philippine Currency
per month, payable on the 15th and 30th day of each month thereof, and shall
also pay the Second Party a ten percent (10%) commission on all services
rendered, such commission to be paid within ____ days after the monthly
accounts have been certified by the First Party, which account shall be
conclusive. Subject to the determination of the First Party, she shall also pay
the Second Party an additional monthly bonus of FIVE HUNDRED PESOS
(Php 1,500.00) Philippine Currency in the event that the business shall earn
a monthly service sales of more than ONE HUNDRED EIGHTY THOUSAND
PESOS (Php 180,000.00) Philippine Currency or
_________________________________ (Php ______) Philippine Currency
for monthly service sales exceeding TWO HUNDRED THOUSAND PESOS
(Php 200,000.00) Philippine Currency;
3. The Second Party shall be entitled to fifteen (15) days sick and fifteen (15)
days vacation leave, each year provided that notice shall be served upon the
First Party when such leave shall be taken, with the vacation leave to be
made known at least three (3) days prior to the date of leave thereon;
4. That the Second Party shall well and faithfully serve the employer in such
capacity as aforesaid, and shall at all times devote his whole time, attention
and energies to the services of the business to the best of his ability, and
shall do perform services, acts and things connected therewith as the First
Party shall from time to time direct, and are of a nature properly belonging to
the duties of Assistant ;
5. It shall be understood that the duties of the _Assistant_ shall only be of a

service nature and that he/she shall not be held accountable in any manner
whatsoever for the property or funds belonging to the business, it being
incumbent upon the employer to employ a bookkeeper and a cashier whom
she trusts;
6. Either party may terminate this contract without cause by giving to the other
one (1) month’s notice in writing, but without prejudice to any right or claim,
which, as of that date, may have accrued to either of the parties hereunder.
In the event of breach of this contract, the employer may terminate this
agreement without notice or payment of salary or commission as hereinafter
provided.

1. The Second Party, subject to the discretion of the First Party, may be
entitled to a cash advance equivalent to three (3) months salary,
payable within one year plus one percent (1%) monthly interest, which
shall be deducted from his/her monthly compensation; (this stipulation
