Duration of Fact assessment: From (d/m/yr) _______________To: (d/m/yr) __________
Team of assessor: ___________________ Address: ________________Phone: ____________

1. Personnel requirement

1.1 What are the Criteria for selecting of laboratory personnel?

 Educational background
 Appropriate training on the GLP and specific tasks s/he assigned to
 Certificate of competency

1.2 How an accredited laboratory structures be organized ?

QMR

Custodian
Chemical store person
Maintenance and calibration facilitator

1.3 How to get short term training for lab personnel

 Not clear; but

 If your request is to know who and how the short term training is to be managed once you are
engaged in accreditation process:
o QMR will be responsible to plan training to lab personnel
o Management commitment in facilitating the training is mandatory
o Trainings can be arranged with ENAO and ESA; and there are very relevant training
topics that you should be able to identify before start to planning
 If your interest is to know whether our institute provides short training or not, officially not
possible. We can only share our experience with special arrangement

1.4 What are the criteria of selecting of internal auditor?

 There must be a training given to the candidate internal auditor; and has a certificate of this
training completion
 The personnel should be well experienced in the whole laboratory activities he/she is to be
assigned

 But the internal auditor should not always be a separate staff, rather a personnel who is also
engaged in the routine analysis; in which case more than one internal auditor is required to
check and balance

2. Work Environment

2.1 What is the requirement for proper lab work environment? like area layout , water supply system ,
waste disposal system
 It all depends on the parameter the lab is interested to put in place the accreditation system
for
 Be sure that all the system in the lab and environment doesn’t interfere with the parameter
of focus
 No back flow in the sample management and analysis system (analysis should assure a
unidirectional workflow); and hence your environment/lab layout need to fit to this system
 Waste disposal depends on the types of wastes you have. First you need to know what kinds
of wastes you have, and then develop a manual for waste management depending on the
type and volume of them

3. Quality Management Manual

3.1 who is responsible for preparing quality management manual

 Quality manual can be classified in to two: Management requirement and Technical requirement:
o Technical requirement of the laboratory manual should be developed by group of
personnel who are involved in the analysis and knows the chemistry/biology of the work
o The management requirement part should be prepared by a group of personnel who
knows the college and the laboratory system very well
 In both cases, the document should be authorized by QMR; and those documents which are
categorized as quality policy of the college should be authorized by the highest authority of the
college

3.2. What requirements are needed for preparing quality manual?

 Knowledge and/or enough training on ISO system (ISO 9001 ? ISO17025)

 Motivation/ commitment of the staff members who are assigned to prepare the quality
manual
 Standards and requirements of ISO (the document)
 A person who can guide the team (consultant)
 Good knowledge of reading, understanding and writing English
 Regular review of developed policies, objectives and operational procedures for awareness
creation and public endorsement
 Continuous guidance, support and encouragement of the top management; and regular
approval of documents

4. Equipment

4.1 What are selecting criteria’s for appropriate laboratory equipment?

2|Page
 Calibration result is the most critical deciding tool whether the equipment is usable or not
 It depends on the procedure the laboratory is using. If the procedure mention specific equipment,
then using that equipment might be required unless and otherwise another validation is availed …

5. Chemical and Regents

5.1 How to use, store and dispose chemicals and regents?

 Disposal is not easy in Ethiopia; and much care has to be taken

 I recommend you to adopt some laboratory management tools; like chemical store management
system and more

 If you need more information, please don’t hesitate to ask

Best of luck,
