    This Judicial Affidavit is being offering to show and prove the following:
    a. That affiant is the defendant in this case;
    b. All the allegations in the Petition including all annexes appended thereto and
    which were already marked as exhibits during the Pre-Trial of this case;
    c. All other related matters, facts and circumstances relevant and material to this
    case.

    This Judicial Affidavit was taken at the office of Prosecutor Jet Manuel at the Las
    Pinas Hall of Justice, Las Pinas City.

    Questions were propounded by Prosecutor Jet Manuel and these questions are
    numbered consecutively and each question is followed by the answer of the witness.

    Q: Do you swear to tell the truth and nothing but the truth?
    A: I do.
    Q: Are you aware that you may face criminal liability for false testimony or perjury if
    you will not tell the truth?
    A: I am.
    Q: Please state your name, age address and occupation?
    A: I am Jane Macalino, 25 years old, married, and residing at #8, Via Cosenza St.,
    Las Pinas City
    Q: Do you know the accused in this case, PO1 Kristian Perez?
    A:Yes. He was the one who sexually assaulted me.
    Q: Where were you on the evening of February 16, 2014?
    A: I was drinking with my friends just outside our house.
    Q: What happened thereafter?
    A: When we were about to go home, I saw the accused PO1 Kristian Perez outside
    and called me out, saying that we violated IATF Guidelines, then he approached me
    and tied my hands and forced his penis inside my mouth, he then after threatened to
    kill me.
