    MOTORCYCLE VEHICLE

    I, GENEROSE TRINIDAD GARCIA, of legal age, Filipino,


    and a resident of Sitio Pook Sampaloc 2, Sariaya, Quezon City, after
    having been duly sworn to in accordance with law do hereby deposed
    and stated, that:

    1. I am the owner of Motorcycle Vehicle, with the description


    hereunder:

    MAKE: YAMAHA PLATE Number: 6396WL


    YEAR MODEL: 2010 CHASSIS Number: 40C010550
    MV File number: 0425-00000076120 ENGINE NO. 0425-00000076120

    2. The said motorcycle vehicle was need to replace its casing


    because of its damage;

    3. In this incident, I force to make-over my Motorcycle Vehicle


    and change the color from BLACK to BLUE; and

    4. That executed this affidavit to attest to the truth of the


    foregoing set of facts.
