    NOTICE OF AWARD (NOA)

    Dear Ms.
    MONDIDO:

    We are pleased to inform you that you qualified as a UniFAST-TDP grantee with Award No. CHEDTDP-001496-17-01. This
    grant is effective 1st Semester, AY 2020-2021 until 2nd Semester, AY 2020-2021 with Seven Thousand and Five
    Hundred pesos (P 7,500.00) only per semester.

    Please be informed that this Notice of Award means that the grantee is a Senior High School graduate; or an ongoing college
    student with earned units in CHED-recognized programs of Private HEIs, SUCs or LUCs with Certificate of Program
    Compliance (COPC) with at least a passing general weighted average (GWA); or a passer of the Alternative Learning Systems
    (ALS)/ Philippine Educational Placement TEST (PEPT).

    You are advised to constantly coordinate and communicate with UniFAST Regional Coordinator/HEI, regarding any concern
    with regards to your grant. Further, please notify the Regional Office/HEI within 15 days from occurrence about any change
    of permanent addresses and contact numbers.

    Grounds for Termination based on Section 12 of UniFAST Memorandum Order No. 02 Series of 2021,
    failure to:

    Failure to confirm acceptance of the award within 15 calendar days from receipt of NOA
    Failure to maintain a GWA of at least a passing grade, regardless of the status of grades per subject;
    Dropped out from the HEI;
    Failure to enroll a regular load per term, as determined and approved by HEls;
    Failure to secure approval from concerned CHEDRO deferment of the grant, or transferring to another program or
    HEI;
    Submission of falsified or fake documents; and
    Availment of multiple national government-funded assistance, except for Free Higher Education in SUCs and LUCs
    under R.A. 10931 or the Universal Access to Quality Tertiary Education Act
