    AFFIDAVIT OF TWO DISINTERESTED PERSONS


    (One and the Same Person)

    We, ISABEL C. ALMODOVAR and NORMA G. CABILADAS, both of legal ages,


    Filipinos, widows, and residents of and with postal address at Brgy. Baroro, Bacnotan, La
    Union, Philippines after having been sworn to in accordance with law hereby depose and
    state that:

    We personally know ANTONIO C. MANZANO SR., A.K.A. JOSE C. MANZANO, who is


    our long-time neighbor and acquaintance for more than fifty (50) years or so, and the fact of
    his birth being born to the late Sps. Ricardo Manzano and Tomasa Cabagbag-Manzano in
    Brgy. Baroro, Bacnotan, La Union on October 8, 1944 and was registered at the Local Civil
    Registrar’s Office of Bacnotan, La Union as one, ANTONIO C. MANZANO; as per
    Certifications issued by the Office of the Municipal Civil Registrar of Bacnotan, La Union
    dated September 10, 2021 and February 1, 2011 and hereby attached as integral parts
    hereof as Annexes “A” and “B”, respectively

    We also know that during his marriage to Tomasa Cabagbag, they had a son whose
    name was registered as one, ANTONIO B. MANZANO, JR., and as a consequence of which
    ANTONIO C. MANZANO became one, ANTONIO C. MANZANO SR.;

    After the birth of his son, ANTONIO JR., ANTONIO C. MANZANO assumed and
    consistently used the name, ANTONIO C. MANZANO, SR., in both his personal and
    business undertakings as evidenced by his Senior Citizen Identification Card bearing serial
    numbers 003090 issued by the Office of Senior Citizens Affair of Bacnotan, La Union LGU
    issued on March 27, 2018 and hereby made an integral part hereof as Annex “C”;

    We also know that when ANTONIO C. MANZANO A.K.A. JOSE C. MANZANO SR died
    on August 11, 2021, his Certificate of Death listed him as one, JOSE C. MANZANO, SR;
    and a copy of which is hereby attached as Annex “D” and hereby made an integral part
    hereof;

    We attest to the fact that JOSE C. MANZANO SR., A.K.A. JOSE C. MANZANO is one
    and the same person, the son of the late Sps. Ricardo Manzano and Tomasa Cabagbag-
    Manzano; and father to one, ANTONIO B. MANZANO, JR., and one, ABNER B. MANZANO

    We are not in any way related to the foregoing person within the fourth civil degree
    either by consanguinity or affinity, nor do we have any pecuniary interest in the subject
    matter for which this affidavit may be used and/or introduced; and

    We are executing this affidavit to attest to the truthfulness of the foregoing statements
    and for all legal intents and purposes it may serve.

    IN WITNESS WHEREOF, WE hereby set our hands this 10TH day of January, 2022 at
    San Juan, La Union, Philippines.
