    REASON(s) / JUSTIFICATION(s)
    IN CHOOSING THE TOPIC:
    One of the primary concern most of the staff of guidance

    department and teachers are to observe the behavior of the

    student inside the classroom, to monitor daily the attendance of

    the student and conduct faculty evaluation at the end of every

    semester. As time goes by, the Center for Guidance Services

    Department of Our Lady of Fatima University implemented that

    the faculty must fill up and submits the "Student Referral Form"

    every cut-off period. It is hard for the faculty to fill-up the referral

    form for 1 to 2 hours preparation. It includes the name of the

    student, number of absences, date of absences, contact number,

    address, and remarks. They are required to submit the said form

    together with their "Daily Time Record" summary of every cut-


    off.

    The proponent would like to design a system that would

    provide faculty evaluation results such as the faculty’s

    performance per semester with its equivalent satisfactory level of

    teaching and its strength and weakness, and at the same time the

    system would record and monitor the attendance of the student.

    The propose system is entitled "WEB-based Faculty Evaluation

    with Student Attendance Referral Monitoring System for Our

    Lady of Fatima University (Antipolo Campus)" since Our

    Lady of Fatima University is one of the most largest school in the

    Philippines with regards to the enrollment population. It also has

    four branches located in the Valenzuela, Quezon City, Antipolo

    and Pampanga branch. Other than these the University offers

    various courses like nursing, dentistry, business administration,

    marine, criminology, information technology, hotel and restaurant

    management and travel management. In this course, the guidance

    staff and all faculties need to monitor the attendance of their

    respective students by section.

     The study will focus on guidance services.
