CLASSIFICATION GI stimulant

DOSAGE AND FREQUENCY 10mg, IVTT q8

MECHANISM OF ACTION Stimulates motility of upper GI tract, increases

lower esophageal sphincter tone, and blocks

dopamine receptors at the chemoreceptor

trigger zone

INDICATIONS Relief symptoms of acute and recurrent

diabetic gastroparesis, short-term therapy for

adults with symptomatic GERD who fail to

responds to conventional therapy

CONTRAINDICATIONS hypersensitivity; use cautiously in patient with

history of depression, Parkinson’s disease or

HTN

SIDE EFFECTS nervousness, dizziness, restlessness, confusion,

trouble sleeping
ADVERSE EFFECTS CNS: anxiety, drowsiness, dystonic reaction,

fatigue, restlessness, seizure, fever,

hallucination, headache, insomnia, dizziness

CV: bradycardia, supraventricular tachycardia,

hypotension, transient HTN, HF

GI: bowel disorder, diarrhea, nausea

GU: incontinence, urinary frequency, erectile

dysfunction

Hematologic: agranulocytosis, neutropenia

Skin: rash, urticaria

NURSING RESPONSIBILITIES 1. Monitor the bowel sounds of the patient

2. Monitor patient for dizziness, headache,

or nervousness after the drug is

stopped; these may indicate withdrawal

3. Monitor patient for fever, CNS

symptoms, irregular pulse, cardiac

arrhythmias or abnormal BP which may

indicate NMS

4. Tell patient to avoid activities that

require alertness for 2 hours after doses

5. Tell patient to report persistent or

serious adverse reaction promptly


6. Teach patient signs and symptoms of

tardive dyskinesia, other

extrapyramidal signs and symptoms

and NMS

7. Advise patient to discontinue drug and

to seek immediate medical attention if

sign and symptoms of tardive

dyskinesia occur

8. Advise patient not to drink alcohol after

therapy

9. Monitor for possible hypernatremia and

hypokalemia, especially if patient has

CHF or cirrhosis

10. Report immediately the onset of

restlessness, involuntary movements,

facial grimacing and tremors.
