1. Researching medications for drug cards prior at least one day before clinicals beginning each Thursday and Friday.
2. Conduct research on patient diagnoses and understand the link between them and the use of medications after each patient

I will achieve this by:

1. Reviewing my patients' profile on EPIC at the beginning of each clinical shift to ensure that I am prepared to provide care tailored to
their conditions and preferences.
2. I can review notes that have been made by health care professionals to gain a richer understanding of my patients' health history and
evaluate for required changes in daily care (Ex. Ambulation, nutrition, mental health, etc.) throughout each clinical shift.
3. I will document vital signs, care, and any assessments for patients in a timely manner to promote good communication among
health care professionals and accurate findings for future reference during my clinical shifts.

Throughout my clinical I learned about medication administration, reports,


and documentation in greater depth. At first I was a little intimidated about
giving report, however through more practice I was able to develop stronger
communication skills about my patients. Overall, my Learning Plan helped
me identify and target areas that I wanted to improve on.
