QUITCLAIM

3. I completely understand the consequences of may termination due to the nature of my
employment which is only a project based. That I was accepted by the company and I Agreed to
be employed only for a specific duration of the project or while my service is needed for the
project.
(Naiintindihan ko ng maayos ang naturang kadahilanan ng pagtatapos ng aking kontrata at ang
mga naging bunga neto, dahil sa ako ay tinangap ng kompanya at pumayag na magtrabaho
para lamang sa proyekto at habang ako ay kailangan lamang sa proyekto.
4. I hereby acknowledge to have received from _______ the sum of

which represents as full and final satisfaction of my salary and other benefits and accrued leave
pay, that may be due to me under existing laws and contract, for the service which I have
rendered for the Company;

kumukatawan ng buo at huling pagpabayad ng aking sahod at benepisyo and accrued leave
pay, na naaayon sa batas, aking kotrata sa buong panahon ng aking pagtrabaho sa Kumpanya;)
5. Consequently, I hereby release and forever discharge _________, its employees, stockholders,
officers, members of the Board of Directors, agents and successors-in-interest (collectively the

“Released Parties”) from any and all obligations, claims, demands, liabilities or cause of action of
whatever nature arising out of my employment even with respect to injuries, illnesses, rights and
claims not mentioned herein;
(Dahil dito, pinapawawalang-sala at habang buhay na pinalalaya ko ang __________, mga
empleyado nito, may ari, opisyal, Board of Directors, ahente, at mga tagapagmana nito (sama-
sama na “Mga Pinalalaya”) sa anumang obligasyon, kahilingan at demanda, pananagutan at
sanhi ng aksyon na nag-uugat sa aking pagiging empleyado nito kasali na kasali na pati ang
mga pinsala, sakit, karapatan at paghahabol na hindi nabanggit dito;)
6. I further agree that this RECEIPT, RELEASE AND QUITCLAIM may be pleaded to bar any suit
or proceeding to which either I, or my heirs and assigns, may have against the Release Parties;
(Ako ay pumupayag rin na ang PAGTANGGAP, PAGPAPALAYA AT PAGPAPAUBAYA na ito
ay maaaring gamitin upang ipawalang-bisa ang anumang kaso o asunto na maaring maisagawa
ko o ang aking mga kamag-anak laban sa Mga Pinalalaya;)
7. I agree that this RECEIPT, RELEASE AND QUITCLAIM shall in all respects be governed by the
laws of the Philippines including all matters of construction and validity. I further agree that any
disputes arising from this document shall be referred to the competent court or forum of the
Philippines, to the exclusion of any other.
(Pumapayag ako na tanging batas lamang ng Pilipinas ang maaring gamitin sa kahit ano mang
katanungan tungkol sa PAGTANGGAP, PAGPAPALAYA AT PAGPAPAUBAYA na ito, kasama
na ang mga tanong tungkol sa kahulugan at katotohanan ng naturang dokumento. Pumapayag
ako na anumang pagtatalo na nag-uugat sa kasulatang ito ay maari lamang isangguni sa
tamang korte o pagtitipon sa Pilipinas at hindi kung saan mang iba;)
8. I agree that in case I violate the terms and conditions of this RECEIPT, RELEASE
AND QUITCLAIM, willfully or otherwise, I may be held liable for breach of contract and for
payment of damages;
(Pumapayag ako na kung sakali na lalabag ako sa mga probisyon at kondisyones ng
PAGTANGGAP, PAGPAPALAYA AT PAGPAPAUBAYA na ito, maging ito man ay kusang-loob
o sa kahit anupamang dahilan, maari akong managot sa paglabag ng kontrata at sa
pagbabayad ng kaukulang danyos;)
9. I understand and concede that payment under this RECEIPT, RELEASE AND QUITCLAIM by
__________ is not an admission of liability on the part of the Released Parties.
(Naiintindihan ko at pumapayag ako na ang pagbabayad sa ilalim ng PAGTANGGAP,
PAGPAPALAYA AT PAGPAPAUBAYA ng __________________., at ng kompanya ay hindi
tanda ng pag-amin ng kasalanan o pananagutan ng Mga Pinalalaya;)
10. I guarantee that I have fully understood the terms and conditions of this RECEIPT, RELEASE
AND QUITCLAIM. The consequences, results or effects was explained to me in Filipino and I
voluntarily and freely execute it, no agreement, statement, representation, promise or

inducement of any kind having been accepted by me except as is embodied herein.
(Ginagrantiya ko na naiintindihan ko nang lubos ang mga terminolohiya at kondisyon na
nasasaad at napapaloob sa PAGTANGGAP, PAGPAPALAYA AT PAGPAPAUBAYA na ito.
Ang mga ibig saibihin at epekto ng dokumentong ito ay ipinaliwanag sa akin sa salita sa wikang
Filipino, at ito ay aking naiintindihan at nilagdaan ng kusang-loob - na walang pananakot,
pananakit, panloloko, panlilinlang o panghihikayat mula sa anumang paraan na makakaapekto
sa aking boluntaryo na pagpirma nito;)
