    AFFIDAVIT OF LOSS

    I, ___________________________, of legal age and residing at


    _______________________, under oath, state:

    1.       THAT on November 2, 2021, I received the original copy of


    Collection Receipt No. _____________ from
    ___________________________ dated November 2, 2021, indicating
    therein the amount of TWENTY THREE THOUSAND TWO HUNDRED
    THREE PESOS and 13/100 (Php 23, 203.13) under the name of
    ________________________________;

    2. That the same Collection Receipt was attached to a Letter


    Requesting for the issuance of BIR Form 2306 and 2307, and
    documents for reimbursement of bond;

    3. THAT the aforementioned Collection Receipt, Letter Request, and


    documents for reimbursement of bond were placed inside a long white
    envelope and sealed inside a parcel addressed to
    _________________________________________________________;

    4. THAT such parcel containing the Collection Receipt, Letter


    Request and documents for reimbursement of bond were sent thru LBC
    ____________ Branch on November 4, 2021, with tracking number
    _____________________;

    4. That when the parcel arrived at the ________________________


    office on November ____ 2021, it no longer included the aforementioned
    Collection Receipt and Letter Request;

    5. THAT I exerted diligent efforts to locate the said documents and


    trace its whereabouts but to no avail and I therefore consider the same
    as lost and cannot be found;

    6. THAT I am executing this Affidavit to attest to the truth of the


    foregoing facts and to use the same for whatever legal purpose it may
    serve.
