    AFFIDAVIT

    I, MARITES SANTOS CUI, of legal age, Filipino, with residence and


    postal address at No. 74 PRK 2, Brgy., Dontogan, Km. 5 Green Valley,
    Baguio City, after having been sworn in accordance with law, hereby
    deposes and says that:

    1. I am currently processing my application as an English Teacher


    to work abroad;

    2. Among the requirements needed for the said job application


    are necessary trainings and certificates related to teaching;

    3. On 7 December 2021, I have enrolled and completed 120 hours


    of training for TEACHING ENGLISH AS A FOREIGN LANGUAGE (TEFL)
    offered by TONI VANILLI ENTERPRISE (TVE) Coaching Enterprise with
    office address at Room 219, 2/F, Victoria Supermart Bldg., Upper Mabini
    Street, Baguio City Philippines;

    4. As proof of the above-said training, a copy of the Certificate of


    Completion of the course: TEACHING ENGLISH AS A FOREIGN LANGUAGE
    (TEFL) issued to me by TONI VANILLI ENTERPRISE (TVE) Coaching
    Enterprise is hereto attached;

    5. I executed this affidavit to attest to the truth and veracity of all


    the foregoing facts and for all other legal intents and purposes.
