1.5” SPM Valves
Date: July 28, 2014

Subject: 1.5” Normally Open Bidirectional SPM valves located on the rigid conduit
manifold used as rigid conduit isolation or pod supply. Some BOP stacks have
LMRP accumulator charge plumbed directly off of the rigid conduit. These
valves are also subject to this PIB.

Product: 1.5” SPM valve (NOV part numbers 7401780, 20024781, 10684103-001, and
10722564-001)

Affected
Assemblies: 5th Generation Multiplex BOP Control Systems

Objective: To inform the industry of potential SPM valve upper seal plate failure to the
rigid conduit isolation, pod supply, or LMRP accumulator charge (if located
directly off of the rigid conduit manifold) when deploying the LMRP.

Issue: The SPM valves located in the above referenced locations are configured as
normally open valves without a vent in the SPM block. When deploying the
LMRP, one of two scenarios can be present with respect to each valve in
question. The first scenario is that the LMRP is deployed with the valve in the
closed position (Scenario 1). The second scenario is that the LMRP is
deployed with the valve in the open position (Scenario 2). There is risk of
damage to the upper seal plate in both scenarios if the valve is not cycled
every 1,000 feet.

Scenario 1: Deploying the LMRP with the SPM Valve in the Closed Position

Deploying the LMRP with the valve in the closed position can result in trapped
atmospheric pressure in the SPM valve. Since there is no vent port in the valve
block, the trapped pressure has no way of equalizing with the surrounding
hydrostatic pressure as the LMRP is lowered.

As the LMRP descends, hydrostatic pressure increases around the valve and
is allowed to act on the seawater side of the upper seal plate through the vent
ports in the SPM housing. As hydrostatic pressure continues to increase, the
differential pressure across the upper seal plate can get large enough to cause
it to fail.

Scenario 2: Deploying the LMRP with the SPM Valve in the Open Position

Deploying the LMRP with the valve in the open position can result in trapped
atmospheric pressure in the upper cavity of the SPM valve surrounding the
spool. Since there is no vent port in the valve block, the trapped pressure has
no way of equalizing with the surrounding hydrostatic pressure as the LMRP is
lowered.

As the LMRP descends, hydrostatic pressure increases around the valve and
is allowed to act on the seawater side of the upper seal plate through the vent
ports in the SPM housing. As hydrostatic pressure continues to increase, the
differential pressure across the upper seal plate can get large enough to cause
it to fail. However, the failure will not occur until the valve is switched to the
closed position. As the spool travels to the closed position, the upper seal plate
deflects due to the low pressure surrounding the spool. If the upper seal plate
deflects far enough, it will fail.

Solution: If Deploying the LMRP with the SPM Valve in the Closed Position

Short Term Fix – During stack deployment, individually function each rigid
conduit isolation, pod supply, and LMRP accumulator charge (if located
directly off of the rigid conduit manifold) SPMs to the open position for 15
seconds then return the SPMs to the closed position. This should be done
every 1,000 ft. during deployment of the LMRP.

Long Term Fix – Service the installed 1.5” normally open rigid conduit
isolation, pod supply, and LMRP accumulator charge (if located directly off
of the rigid conduit manifold) SPMs (NOV P/N 7401780, 20024781,
10684103-001, and 10722564-001) with NOV Conversion Kit P/N
10916753-001. The conversion kit includes the new integrated backup plate.
This plate replaces the old upper seal plate (NOV P/N 1006582) and backup
plate (NOV P/N 1006603), by combining the two into one integrated metallic
backup plate (NOV P/N 10916738-001). For NOV P/Ns 7401780 and
20024781 replace the spool (NOV P/N 1006537) with the pressure balanced
spool (NOV P/N 10684118-001) found in NOV Repair Kit P/N 10925328-
001. After installation of the kit (or both kits in the case of NOV P/Ns
7401780 and 20024781), use an engraver and modify the NOV P/N located
on the SPM housing. If the valve has NOV P/N 7401780 or 10684103-001,
scratch it out and engrave 10916756-001. If the valve has NOV P/N
20024781 or 10722564-001, scratch it out and engrave 11008619-001.
Once the long term fix has been applied, disregard the steps outlined in the
short term fix.

Recommendation:

Until it is convenient to modify the identified SPM valves on the LMRP, cycle
the identified valves every 1,000 feet during deployment of the LMRP as
stated in Short Term Fix under the appropriate section in the Solution portion
of this document. This will allow pressure within the SPM to equalize with
the surrounding hydrostatic pressure and prevent upper seal plate failure.

When it is convenient to modify the identified SPM valves on the LMRP,

replace the affected parts of the identified SPM valve with those included in
the conversion kit listed in the Long Term Fix.
