1. 1. Verify order.

2. Wash Hands.
3. Introduce yourself, your role, purpose of your visit. Identify
your client using 2 identifiers. Explain the procedure to the
client..Assess for level of post-operative pain. Give PRN
medication for pain to make procedure pain-free.
1. 4. Prepare a wound care tray with the following:
2. New bandages, forceps
3. A roll of medical tape
4. Clean scissors
5. Two pairs of disposable gloves
6. Two, clean, soft cloths or paper towels
7. A disposable plastic bag
8. Waste basket
9. Drape for client (optional)
10. Underpads

5. Cut four pieces of micropore tape and hang them from a nearby
table, the pieces of tape should be the same length as the old ones
around the incision.

7. Pour cleansing solution on sterile tray
8 Prepare patient and expose dressed incision site
9.Do hand hygiene and wear disposable gloves

11. Assess the old dressing for color, quantity, type of drainage
(foul, sweet smell, yellowish or greenish stain may mean the
presence of infection-if present report to physician/surgeon
immediately)

12. Place old dressing into the disposable plastic bag

13.Remove inner dressing with transfer forcep


14. Assess inner dressing for foul smell and stain and discard into
the disposable bag

15.Discard transfer forceps and nonsterile gloves according to


agency policy

16.Place water resistant underpad

17. Do hand hygiene and don clean gloves

18.Cleanse the wound (use sterile forcep) using sterile 2x2


gauze/swab per stroke from top to bottom or from incision to
edges
-using sterile swab clean the suture line by starting at the center
and working towards one end, discard the swab
- using another sterile swab clean the suture line by starting at the
center and working towards one end, discard the swab
-all other cleansing involves moving from one end to the other end
on ach side of the incision
-work in straight lines, moving away from the suture line with
each successive stroke

19.If a drain is present, cleanse around the drain using circular


stroke starting with the area immediately next to the drain

20. Using another swab, cleanse immediately, next to the drain


and attempt to clean a little further out from the drain. Continue
this process with subsequent swabs until the skin surrounding the
drain is cleaned.

21.Apply inner dressing (4x4 gauze) with the use of the forceps
placing them into the incision site and drain site(if present)

22. Discard non sterile gloves into the disposable plastic bag
nearby

 23.Apply outer dressing and tape all sides

 24.Place abdominal binder snuggly around the client

 25. Assist patient to comfortable position, lower patient’s


bed, put side rails, place call light near client, discard used
equipment appropriately and perform hand hygiene.

 26. Document.
