RE: Religious Exemption from Immunization Requirements:

I, Carolyn Y. Rhoe the undersigned, do hereby affirm that I am a member of the Moorish
Science Temple of America Inc. Under the divine teachings of Prime Minister NGSKS:
Durriyyah Bey. I, Carolyn Y. Rhoe am exempt from the immunizations required by Atlas
Foundations of Washington LLC due to my religious tenets and practices. On this basis, as the
above-referenced immunization requirements violate my right to freely exercise my religion as
guaranteed by the First Amendment of the Constitution of The United States of America, I am
asserting my rights to an exemption from Atlas Foundations of Washington LLC immunization
requirements.

I, Carolyn Y. Rhoe qualify for this exemption based on the following:

1) In the Islamic faith under the Moorish Divine and National Movement religious creed we do
not put foreign matter or animal products into our forms for medical or non medical reasons.
We are not scientific experiments we are human beings who willing serve our creator. To inject a
foreign matter into our form is a violation natural law, which causes chaos in the temple aka
body that we willing serve our creator with. Our forms are temples of the creator and therefore
must remain clean from all manmade and animal products. We only sacrifice for Allah (God) the
Creator we serve.

2) Religion is a God given right which allows people of faith to have the freedom that First
Amendment of the United States Constitution Amendment, 1, and Article 18 of the International
Freedom Act of 1998. Everyone has the right to freedom of thought, conscience and religion;
this right includes freedom to change his religion or belief, and freedom, either alone or in
community with others and in public or private, to manifest his/her religion or belief in teaching,
practice, worship and observance up holds.

3) Title VII of the Civil Rights Act (42 U.S. Code § 2000e) prohibits discrimination against a
sincerely held religious belief, practice, or observance. As enforced by the U.S. Equal
Employment Opportunity Commission, Title VII requires employers to reasonably accommodate
an employee's sincerely held religious belief, practice, or observance. For religious
accommodation requests, according to the EEOC's COVID-19 guidance, employers should
ordinarily assume that an employee's request for religious accommodation is based on a sincerely
held religious belief, practice, or observance.
EEOC guidance also reminds employers that the ADA prohibits employers from both disclosing
that an employee is receiving a reasonable accommodation and retaliating against an employee
for requesting an accommodation.

Failure to uphold 42 U.S. Code § 2000e and any attempt to coerce an individual to get the Covid-
19 vaccine is a violation of federal law and may be met with lawful action.
