PETITION
COMES NOW PETITIONER, by himself and unto this Honorable

Court, respectfully alleges:
1.) That he is a Filipino citizen, of legal age, married, born on May

26, 1979 and a resident of 1272 Crosan, Bulacao, Talisay City, Cebu,
Philippines, since birth, with telephone number (032)511-4023.
2.) That he is a member of the Philippine Bar in good standing,

having passed the Bar Examinations given by the Honorable Supreme Court
in September 2006 and admitted to the practice of law on April 26, 2007;
3.) That as member of the Philippine Bar, Petitioner signed in the

Roll of Attorney's on April 26, 2017 under Roll No. 53097;
5.) Petitioner paid his Professional Tax for the current year under
Official Receipt No. ________________, issued on _________ at Talisay
City. A copy of which is hereto attached as Annex “A”;
6.) That he possesses all the qualifications and none of the

disqualifications for the Office of the Notary Public and thus, he desires to
be commissioned as Notary Public for and in the City of Talisay, City of
Naga and the Municipality of Minglanilla, Cebu under the jurisdiction of the
Honorable Court;
7.) That he has never been convicted of any crime involving moral
turpitude, or the like. Petitioner likewise has no pending Administrative case
before the Office of the Bar Confidant, Supreme Court of the Philippines;
8.) That he has secured the necessary certifications such as the

Certification issued by the Office of the Bar Confidant, Supreme Court,
Manila, copy of which is attached and marked as ANNEX “B”, RTC Talisay
City and MTCC Talisay City Clearances which are hereto attached as Annex
“C” and “D”, respectively, NBI Clearance which is hereto attached as
Annex “E”; and Certificate of Good Moral Character issued by the IBP –
Cebu City Chapter, copy of which is attached and marked as ANNEX “F”,
National Bureau of Investigation Clearance, copy of which is attached and
marked as ANNEX “G”;
9) That petitioner has been granted a Notarial Commission and has

been a Notary Public in the City of Cebu, City of Talisay, Cebu,
Municipality of Minglanilla, Cebu, City of Naga, Cebu, Municipality of San
Fernando, Cebu and City of Carcar, Cebu, since 2007 and his Notarial
Commission is set to expire at the end of this year. The renewal of his
Notarial Commission is allowed by law provided it will be filed within 45
days before its expiration. That he is seeking for a Notarial Commission in
the City of Talisay, Municipality of Minglanilla and City of Naga, taking into
consideration that he has a law office in East Poblacion, City of Naga, Cebu;
10.) That he is presently holding a law office at East Poblacion, City

of Naga, Cebu. That in the event of change of office address, petitioner
undertakes to inform the Executive Judge of this Honorable Court within ten
(10) days thereof;
11.) That attached hereto is a separate sheet of paper containing (3)

specimen signatures of the petitioner, copy of which is attached and marked
as ANNEX “H”;
12.) That attached hereto are copies of a passport size photograph of

petitioner duly signed by him at the bottom thereof, copy of which is
attached and marked as ANNEX “I”.
PRAYER
WHEREFORE, Petitioner most respectfully prays of this Honorable

Court to ISSUE a Letter of Commission as Notary Public for and in the City
of Talisay, City of Naga and the Municipality of Minglanilla, Cebu under the
jurisdiction of this Honorable Court.
Other reliefs just and equitable under the premises are likewise prayed
for.

proceeding involving the same party, issues and subject-matter in the
Supreme Court, the Court of Appeals, or any other tribunal or agency, and
that no such other tribunal or agency, and that no such action or proceedings
pending before any of the foregoing;
4.) If I should learn hereafter that a similar action or proceedings has

been filed or is pending with the Supreme Court, the Court of Appeals, or
any other tribunal or agency, I undertake to report that fact to this Honorable
Court within five (5) days from knowledge thereof.
