FACTS OF THE CASE

On March 07, 2019 at about 4:00 in the afternoon at Purok


Upper, Brgy. San Vicente, Baguio City, a concerned witness Mrs. Ruth
Canal, 52 years old, married, housewife, presently residing at #262
Purok Upper, Brgy. San Vicente, Baguio City reported at BCPO
Station 8, Baguio City regarding a fire incident that transpired on the
foresaid place of incident wherein the house owned by Mr. Jack K.
Ulero, 38 years old, single, security guard was razed by fire.
According to the witness, before the incident, she saw a man
holding a small bottle and spraying its contents on the wall of the
house. The witness simply thought that the man intended to clean or
replace the wall varnish. At first she did not doubt the actions of that
man because she knew this man was none other than Mr. Villamore
T. Pastor, single, 38 years old, vendor, and presently residing near the
said barangay at #284 Lower Balsigan, Baguio City who was known
as a childhood friend of the house owner. Later she noticed where
the thick smoke was coming from and looking at said house it was
already burning.

Upon receipt of the information, elements of BCPO Station 8,


Baguio City relayed the information to BFP Baguio. Initial inquiry
conducted BCPO Station 8 to FO3 Gene F. Mendoza, Arson
Investigator of BFP Baguio City disclosed that the cause of fire was
due to intentional application of accelerant found in the area of
incident. Fire out was declared at about 4:25 PM of same date
without reported casualty/injured person during the incident while
estimated cost of damages still undetermined.

I, Villamore T. Pastor, voluntarily and consciously agree to be interviewed and to


undergo polygraph examination by examiners of Polygraph Division/Section PNP Crime
Laboratory. I understand that I am entitled to the following rights in the conduct of
polygraph examination:

1. I may refuse to be interviewed or examined and, if so, I hereby authorize the


Examiner/ Technician PNP Crime Laboratory to inform PCMS. MANNY K. URISTA of
my refusal.

2. I may consult a lawyer, my parent(s), relatives or guardians prior to the interview


and polygraph examination.

3. The interview / and or polygraph examination will be terminated at any time I


upon my request and, if so, I hereby authorize Polygraph Division/ Section PNP
Crime Laboratory to inform the requester of my request.

4. All questions to be asked in the polygraph examination will be discussed with me


prior to the actual testing.

5. No questions pertaining to sexual conduct, obscene or offensive to chastity, or

SECTION 1: PURPOSE OF EXAMINATION

The main issue under consideration for the polygraph examination was whether or not
the examinee was telling the truth to the pertinent questions listed under Section 3 of
this report

SECTION 2: PRE-TEST INTERVIEW

On MARCH 11, 2019, MR. VILLAMORE T. PASTOR arrived and voluntary submitted to a
polygraph examination. The following were read, completed, and voluntary signed by
the examinee:

 Personal Data of the Subject

During the pre-test interview, MR. VILLAMORE T. PASTOR read the contents of the above
forms which he/she understood, signed and agreed to the test, waiving the rights set
forth in the documents. The examinee was fully advised that the entire examination
would be recorded using both audio and video. In the opinion of the examiner, MR.
VILLAMORE T. PASTOR was suitable for polygraph testing.

On the pre-test interview, the examiner discussed and thoroughly reviewed all the test
examinee an opportunity to ensure that he/she completely understands the questions
before the onset of the testing phase.

SECTION 3: IN- TEST PHASE

A Lafayette Computerized Polygraph System, Model LX4000 was used for the collection
of polygraph test data. This instrument makes a continuous recording of autonomic
responses associated with respiration, electro dermal activity, and cardiovascular
functioning. The instrument also includes activity sensors designed to record peripheral
behavior activity and cooperation during the examination. A functional check prior to
the examination confirmed the instrument was in proper working order.

The following Relevant questions were asked during the Polygraph examination.

 Relevant Question 1: “You were there at the scene before the incident
happened weren't you?”
 Relevant Question 2: “Is it true that the owner of the burned house Mr. Jack K.
Ulero is your friend?”
 Relevant Question 3: “Did you have anything to do with the burning of Mr. Jack’s
house?”
 Relevant Question 4: “Do you have a dispute with Mr. Jack?
 Relevant Question 5: “Are you the one who burned the house of Mr. Jack?”

SECTION 4: RESULT___________________________________________________________________

FINDINGS:

 Using the ACEV (Analysis, Comparison, Evaluation and Verification) method,


revealed presence of specific responses of the subject on relevant questions.
Deviations from the normal responses of the subject indicated signs of
deception.

CONCLUSION:

 The subject is deceptive.

REMARKS:
